About the Role

NHC Place At the Trace is seeking a Community Relations Director to develop and maintain strong relationships with targeted referral sources through strategic field call strategies. The role aims to drive and sustain incremental volume in service lines and services, positively representing the organization, clinical service lines, and staff to enhance the center's reputation and market position. The position involves community engagement, marketing, and coordination to support the center's growth and occupancy goals.

Key Responsibilities

  • Initiate and maintain contact with key referral sources in the community using various communication tools to earn referrals to the center.
  • Coordinate admissions to the assisted living and memory care community, including handling inquiries and conducting needs assessments.
  • Share current information about funding resources such as Medicare, Medicaid, Managed Care, and private insurances.
  • Work with residents and families to facilitate adjustment to residential care and address crises related to illness, disability, or death.
  • Serve as an advocate for patients and their families in expressing, defining, and resolving grievances.
  • Handle marketing activities/functions assigned by the Executive Director.
  • Manage stress effectively and respond to community needs promptly.
  • Conduct scheduled face-to-face meetings with targeted referral sources to build trusting relationships.
  • Advance relationships using customized sales strategies and document interactions in the Customer Relations Management Program.
  • Develop annual growth plans based on market share, patient utilization, and competitor analysis.
  • Collaborate with leadership to ensure strategic plans align with facility service lines and growth objectives.
  • Prepare reports, summarize field activities, and identify trends, opportunities, and barriers to new referrals.
  • Coordinate and plan daily pre-admission activities and collaborate with leadership to meet census and occupancy goals.
  • Ensure proper placement of residents in beds, maintaining knowledge of bed availability.

Requirements

  • Preferred Bachelor's degree from an accredited college or university.
  • Experience working with people; sales experience preferred.
  • Ability to read, write, and speak English.
  • Knowledge of the medical case management process, geriatric resident needs, healthcare settings, and funding resources such as Medicare, Medicaid, Managed Care, and private insurances.
  • Creative ability and ability to work cooperatively with professionals.
  • Effective verbal and written communication skills.
  • Ability to exercise independent judgment where procedures cannot be standardized.
  • Willingness to attend meetings, trainings, and conferences, which may require out-of-town travel and overnight stays.
  • Flexibility with weekly work schedule to meet community occupancy goals.
  • Ability to lift 10-20 pounds occasionally, bend, stoop, squat, twist, see and hear adequately, speak clearly, and perform fine motor skills.
  • Ability to work mostly out of the office traveling within a defined geographic area.
  • Ability to work at a fast pace, prioritize multiple activities, and respond promptly.
  • Strong problem-solving skills and creative thinking to navigate complicated situations.
  • Willingness to accommodate referral source schedules outside of standard hours.
